Hyperthyroidism is the most common endocrine disorder in cats, typically affecting cats over 10 years old. An overactive thyroid gland produces excess thyroid hormone, which accelerates metabolism and affects nearly every organ system. The good news: it is very treatable, and most cats make a full recovery with proper treatment. Early detection through regular senior blood panels is key.

Understanding Hyperthyroidism

The thyroid glands (two small glands in the neck) produce hormones that regulate metabolism. In hyperthyroid cats, benign tumors (adenomas) cause overproduction of thyroid hormone. This accelerates metabolism dramatically — causing increased appetite, weight loss, rapid heart rate, high blood pressure, and restlessness. About 98% of cases are benign; only 2% are thyroid cancer.

Symptoms

Classic signs: ravenous appetite with weight loss, restlessness/hyperactivity, vomiting and diarrhea, increased thirst and urination, poor coat condition, rapid heart rate, and sometimes a palpable lump in the neck. Some cats become aggressive or vocalize excessively at night. Because these signs develop gradually, owners often don't notice until the disease is advanced.

Treatment Options

Medication (methimazole/Felimazole): daily pill or transdermal gel applied to the ear. Controls symptoms but doesn't cure the disease. Requires regular blood monitoring. Radioactive iodine (I-131): single injection that destroys overactive thyroid tissue. Cure rate >95%. The gold standard treatment. Requires a short hospital stay at a specialized facility. Surgery: removal of affected thyroid gland. Effective but requires anesthesia. Prescription diet (Hill's y/d): iodine-restricted diet. Effective but limits all other food options.

Kidney Considerations

Hyperthyroidism and kidney disease often coexist in senior cats. Hyperthyroidism can mask kidney disease by increasing blood flow to the kidneys. When hyperthyroidism is treated, underlying kidney disease may become apparent. This is why careful monitoring during treatment is essential — your vet will check kidney values alongside thyroid levels.
