Urinary incontinence — the involuntary leakage of urine — is a common problem in dogs, especially spayed females and older dogs. Causes of Incontinence

Urethral sphincter mechanism incompetence (USMI): the most common cause in spayed female dogs. Decreased estrogen after spaying leads to weakened urethral muscles. Large and giant breeds are over-represented. Neurological disease: spinal cord injury, IVDD, degenerative myelopathy, or nerve damage affects bladder control. Anatomical abnormalities: ectopic ureters (ureters bypass the bladder and connect directly to the urethra), bladder prolapse, or pelvic bladder. UTI: causes urgency and temporary incontinence. Cognitive dysfunction: senior dogs with dementia may 'forget' housetraining.

Symptoms and Diagnosis

Incontinence vs. behavioral urination: incontinence occurs when the dog is relaxed or sleeping — you find wet spots where they were lying. Behavioral urination occurs when the dog is awake and active, often in response to stress, excitement, or marking. Diagnosis: urinalysis and culture (rule out UTI), blood tests (kidney function, glucose for diabetes), neurological examination, and imaging. Cystoscopy (camera in the bladder) identifies ectopic ureters and other anatomical issues. Urodynamic testing measures bladder and sphincter function.

Treatment Options

USMI: phenylpropanolamine (PPA, Proin) tightens the urethral sphincter. Side effects: restlessness, high blood pressure. Estrogen supplementation (Incurin) for refractory cases — used at low doses to avoid bone marrow suppression. Neurological incontinence: depends on cause — some cases improve with treatment of the underlying disease. Ectopic ureters: laser ablation or surgical correction. Pelvic bladder: surgical repositioning. UTIs: antibiotics. Cognitive dysfunction: medications and management strategies. Absorbent pads, dog diapers, and waterproof bedding help manage the home environment.

Living with Incontinent Pets

Management strategies: take dogs out more frequently (every 2-4 hours), use waterproof bedding and furniture covers, dog diapers or belly bands for males, absorbent pads in favorite resting spots, and good hygiene to prevent urine scald (skin irritation from constant moisture). Expressing the bladder manually may be needed for pets with neurological disease. With proper treatment and management, most incontinent pets maintain excellent quality of life.
